My son forgot the iPad password and it is erased. It was set up so long ago that I cannot recall the ID used. How do I reactivate?

You are going to have to try and recover the ID used. If seeing the abbreviated one displayed on the activation screen does not jog your memory, then you will need to take the device and original proof of purchase to a physical Apple Store and request assistance.
