I obtained unlock instructions from ATT for an older iPhone 4 which was being unused after an upgrade. I followed all the instructions in iTunes to back up and restore, and ultimately unlock the phone. However, "restore in progress" was the message of the phone during the process. But, the iPhone 4 never restarted. Any advice?

I see from your post that the restore process has never completed. I suggest that you force your iPhone to restart and then resume the restore process again.
To force restart your device, press and hold both the Sleep/Wake and Home button for at least 10 seconds, until you see the Apple logo.

Does it have to have a SIM card? Because I've tried you suggestion and the phone will not restart.

No, restarting an iPhone does not require a SIM card to be installed. If you cannot get the iPhone to force restart, and it's still showing the Apple logo, I suggest that you get in contact with our Apple Support by using the following link:
