I have gotten mine back when i reboot. I am using an FTDI driver. I am in search of a way for me to get it back without a reboot. When I disconnect my Arduino, the port goes away. If I try the other USB port it comes back, after that I will need to reboot in order to be able to use it again.
Make sure you're running a driver that's the most current and that's appropriate for your particular USB device, then see if there's a specific dismount or disconnect sequence necessary for removing the device, then — if the problem persists – check with whomever has provided the USB driver for suggestions.
The FTDI drivers may or may not work with other USB-to-serial chips, as well. If you're using the Sparkfun FTDI widget or some other widget with the FTDI chip, check directly with the widget vendor.
If you're using OS X 10.9 Mavericks or 10.10 Yosemite or later, see the caveat mentioned at the Sparkfun site, too.
