Playlists are duplicated when syncing

Running the latest version of iTunes (12.2.2.25) on a Mac Min (running Mac OS Yosemite 10.10.5) syncing to an iPhone 6 and new iPod Touch (both running iOS 8.4.1). Syncing everything over Wi-Fi. Having a problem that when I sync music one of my playlists gets duplicated. For example, if there is a playlist in iTunes called "New Playlist", a duplicate playlist called "New Playlist 1" will get created when syncing. I then have to manually delete the playlist then resync to delete the duplicate playlist from iTunes and the device.

Wanted to know if anyone running iTunes on a Mac has experienced the same and of course ideally a known fix.
