On my wife's phone when making call you can't hear the person talking and they can't hear you. She can use Faetime and the speaker works and they can hear her. She can make calls on Facebook Messenger and use the regular speaker and microphone. Why is this not working on regular calls?

I understand that while on a regular call, you are not able to hear from the receiver and the microphone doesn’t pick up sound either. To begin troubleshooting this situation, I would suggest reading over and working through the attached articles below.
