2. It is possible that the SATA cable between the internal disk and the Logic Board is damaged and/or loose. Do you see your internal disk in Recovery Console?
3. If you have an external disk to spare, you can install OS X on an external disk and try to boot from it. Reference - How to install OS X on an external drive connected to your Mac - Apple Support.
4. If you have a second Mac, you can put the bad Mac in Target Disk Mode - Share files between two computers with target disk mode - Apple Support - and then the bad Mac can be diagnosed.
5. If possible, Using Apple Hardware Test - Apple Support, may also provide some useful clues on what might be wrong with your Mac.
