After upgrading to IOS 9 the USB/ Bluetooth only Personal Hotspot will not connect my MacBook Pro. When I select this option with wireless turned off on the iPhone Network Preferences shows not connected.
The Wi-Fi Personal Hotspot works just fine.
Tried restarting both the iPhone and the MacBook.. no help.
For security reasons I prefer to us the USB option. Using a Verizon iPhone 6.

Personal hotspot via USB also no longer works for me. Worked fine in iOS8. Wireless works ok.

Me too. Personal Hotspot works with bluetooth or wifi but not with USB.
I got the same problem both with the iPhone (iPhone 6 Plus) and the iPad (iPad Air 2).
Both were upgraded to iOS 9 and prior to this I know for sure I could use this connection on, at least, the iPad, since I use it daily. I do not have a configuration in the network configs no my Mac for the iPhone since I have never used it on this Mac (Macbook with Yosemite).
The USB is working fine since iTunes and Photos detect both devices once I connect.
The Hotspot is also working fine via WiFi.
I found a post online which refers to a possible solution but I do not have the same errors in the logs to which it referes. Maybe it can be helpful to anyone else.

Tried using my wife's iPhone and IOS 8.4 and the iPhone USB connection does not work anymore either.
It is an iTunes problem not IOS 9.
I guess I'll wait until Apple fixes it.
Same problem with me too. I think this is a bug which needs to be fixed. USB speed is much faster than via bluetooth. Please fix this asap.
I raised a bug report with Apple for this problem. They responded and closed it as a duplicate of another bug (#22740562). So, hopefully, this means we'll see a fix soon.
The upgrade to El Capitan fixed this problem for me.
Me too. I had to uncheck 'Disable unless needed' in System Preferences > Network > iPhone USB to make it work. Not seen this option before. Wonder if it was there in Yosemite and was the original cause of problem? Hmmm.
The problem has returned. Happened again after the IOS 9.0.2 update.
