Q: Iphone 4S upgraded to ios9 but phone locked on Slide to upgrade!!

Iphone 4S was upgraded to iOS9  (confirmed by PC), but, screen is now stuck on "slide to upgrade".

I have already tried the standard hold down on/off switch and app button together. it rebooted and still shows the same message.

Any Ideas?
