Calls for one iPhone are registering on other Iphone call log with iOS9

When I place a call or receive a call on my personal iPhone the call registers on the call log on my work iPhone and visa versa after I update both of the phone to ios 9. Does someone know how to stop this form happening?

iPhone 6, iOS 9, Work Phone is iPhone 5s

MY phones are doing the same thing with my partners phone, people keep telling me to set up family sharing or new iCloud accounts but I have had the same set up for years, this only happened after the iOS 9 update.

Driving us crazy, hope they come up with something soon!!!
