It sounds like you're not seeing your Medical ID on the lock screen of your iPhone 5c, after you updated to the latest iOS 9. The following Knowledge Base article provides some great instruction on verifying that Medical ID shows under "Emergency" on your lock screen:

Medical ID
To make your important health information accessible in case of emergencies, you can personalize your Medical ID on your iPhone.
To add emergency contacts and health information like your birth date, height, and blood type, tap Medical ID > Edit.
Turn on Show When Locked to make your Medical ID available from the Lock screen. In an emergency, this gives people who want to help some important information, like the emergency contacts you've entered.

Dec 30, 2015 9:18 AM in response to pedro dby MartiL2015,I too have this problem. I update medical id data and exit app. When I next access health app all the data is missing. The solution to the problem does not work.
Help please.
