why does my daughters name appear on our itunes home page

Our Apple ID was set up using our 'family' email address and my wife's details. Recently, my daughter 'attempted'? to create an Apple ID for herself, and I think she may have somehow overtyped her name on our ID? What this means for us is that when we log on to iTunes, her name appears on thr home page. Also affects my iPhone as same thing happens and then her music library appears not mine. I have to sign out then sign back in but even doing that sees the account name revert back?
