Exchange Server Sync of Outlook contacts into iPhone 6s creates unwanted email address entries, how to remove?

Exchange Server sync of contacts created multiple email address entries. It appears that certain contacts were ripped apart and separate entries were created for the name and phone numbers and for email address. How can you make sure contacts are imported/sync'd correctly with all relevant contact info under the person's name?
