Hard drive disc isn't available.

MacBook pro 2011 froze a few days after installing Yosemite upgrade online. I can't find the HD drive on disc utility.The first problem I encountered was the time machine didn't backup (immediately after the Yosemite install) I used a USB flash-drive for backup, backups were complete. Is this a hard drive failure?
