icloud account locked and need to log out

my icloud account has been locked and i don't know my password, security questions or the rescue email. i want to log out of it but i cant as turn my iphone is logged in and i cant turn it off as i dont know my password. help me please!

iPhone 5, iOS 9.0.2

You need to contact Apple's Account Security team. To do this, clickhereand pick a method; if that page doesn't list one for your country or you're unable to call, fill out and submitthis form.
