A Few times I have taken photos with my iPhone 6S and the image is really poorly processed or rendered. Is this a hardware issue? Lines blur together and it looks digitized.

Do you have HDR on, and if so, what settings do you have for saving photos?

As far as HDR, what do you have set for the picture to save? HDR takes a series of photos to construct what it believes to be a good one. If you are saving the HDR photo and there is any movement during this phase, you can get this type of behavior. Unless you really need to deal with the things that HDR is made for, you are better off having it turned off.
I have the same problem. Never had this issue with my previous 3 iphones. I already got a new camera at the Apple store, but the issue hasn't been solved yet.
