how do i completely remove the Been Choice app from my Iphone 6

Since Apple has removed the Been Choice app from the app store because of security concerns, how do I now completely remove this app from my Iphone? I installed the app on my phone on 10/7/15

iPhone 6 Plus, iOS 9.0.2

You would remove it the same way as you remove any app -- press and hold the app icon until all of the apps start to jiggle, then tap the x in the upper left corner of the app. Press the Home button to stop the apps from jiggling.
