Bootcamp won't work after installing new partition

Hi. I've been using Bootcamp on my MacBook Pro 9,2. Everything runs great until I decided to create a new OS X partition on my HD. I searched all over the internet how to fix this, but none of the solutions that I found on the Apple Support Communities and other sites solved my problem. I tried deleting the new partition that I created, but the problem still persists. I was thinking of just deleting the Bootcamp partition as well and just install a new one. However, I want to see if it's possible to fix the problem without losing some of my data.

MacBook Pro, OS X Mountain Lion (10.8.2)

Please post the output of the following commands.

diskutil list

diskutil cs list

sudo gpt -vv -r show /dev/disk0

sudo fdisk /dev/disk0

The "sudo" commands will prompt for your password, and it will not be echoed back. You may also see a warning about improper use of "sudo" and potential data loss due to "abuse" of the command.
