support reset ipad 4 when i forget it

i need help when i forget my password ipad 4. i try to remember but i can't.

now i only remember bout id apple and password id apple because i ussualy use it for my iphone.

so, how can i reset password for my ipad when i ony have id apple, i haven't itune...
