    i forgot my firmware password and accidentally formatted my ssd
    now, i cant restore my pc to factory settings and i cant use the pc too
    i need urgent help

    What is your exact computer model? To find the model identifier open System Profiler in the Utilities folder. It's displayed in the panel on the right.

     

    In the future please provide all the relevant information we may need to know such as your computer model, OS X version installed, etc. Put this in your Community profile so it appears at the bottom of all your posts.

    That would seem to be a Mid-2011 model. You will need to reinstall OS X onto the SSD you erased:

     

    Install OS X Using Network Recovery

     

    Be sure you backup your files to an external drive or second internal drive because the following procedure will remove everything from the hard drive.

     

    Boot to the Network Recovery Server:

     

    Restart the computer and after the chime press and hold down the COMMAND-OPTION- R keys until a globe appears on the screen. Wait patiently - 15-20 minutes - until the Recovery main menu appears.

     

    Partition and Format the hard drive:

     

    1. Select Disk Utility from the main menu and click on the Continue button.
    2. After DU loads select your newly installed hard drive (this is the out-dented entry with the mfgr.'s ID and size) from the left side list. Click on the Partition tab in the DU main window.
    3. Under the Volume Scheme heading set the number of partitions from the drop down menu to one. Click on the Options button, set the partition scheme to GUID then click on the OK button. Set the format type to Mac OS Extended (Journaled.) Click on the Apply button and wait until the process has completed. Quit DU and return to the main menu.

     

    Reinstall OS X: Select Reinstall OS X and click on the Continue button. Be sure to select the correct drive to use if you have more than one.

     

    Note: You will need an active Internet connection. I suggest using Ethernet if possible because it is three times faster than wireless.

     

    This should restore the version of OS X originally pre-installed on the computer.

     

    After the installation you will reboot into the Setup Assistant where you can set a new admin password for the new account you will create.

    i tried the network recovery, but it did not work
    it was still asking me for the firmware password

    I don't know if this helps, but restart from your Recovery HD. When the Utilities Menu appears and you can also see the screen menubar, select Firmware Password from the Utilities menu in the screen's menubar. You may be able to remove it or change it. I don't ever use them, so I don't know if you can succeed in getting rid of it.

     

    On the other hand if this is a password for an encrypted drive that you have forgotten or lost, then you are out of luck. You would need to repartition and reformat the drive from scratch.

    i cant restart from recovery HD, it still requires EFI password

    IF you forgot your firmware password only apple can help. They will need to reload the firmware and only Apple has access to the firmware.

     

    Your only resolve is to make an appointment at an Apple Store and they can help. Shouldn't cost too much but you may need to,provide some proof of ownership.

     

    Keep trying to remember that password
