Please download Testdisk and GPT Fdisk and use Boot Camp disappeared after El Capitan update as a reference to run a Testdisk scan and run a Quick Search.

- How do I use Testdisk? I clicked on testdisk and this is what I have:

Testdisk is a Terminal utility. Did you download it from http://www.cgsecurity.org/wiki/TestDisk_Download or somewhere else?
GPT Fdisk is the same way - http://sourceforge.net/projects/gptfdisk/ .
ELC has System Integrity Protection enabled by default. To disable it temporarily for diagnostics purposes, (this is the correct version of the nvram command you referenced earlier).
a. Boot into Local Recovery. Power down your Mac, power it back up, and hold the Command+R.
b. Run Utilities -> Terminal
c. Type csrutil disable
d. Exit Terminal
e. Reboot normally.
I have successfully installed GPT and this is what I have now from Testdisk. Which media should I select?
Please use /dev/rdisk0.

Yes please use EFI/GPT. I had much rather be asked questions than have larger issues come up. Please do not use left/right arrow keys in partition screens.

If you will notice, this is a list of unrecoverable partitions. Press Enter/Return to Continue. You will then see a list of recoverable/viable partitions.

You are not choosing any partition, you are choosing the Continue 'button' at the bottom left of the screen.

Yes. With this specific partition, your partition table looks correct.
Can you use this table and the NTFS Start/Size in steps 7 and 8 in the linked thread (make sure you use the plus sign in step 8) and run the two sets of the Gdisk steps?
