In-named iOS app

Hardware: iPad (Model A1416, 3rd Gen) 64 GB (21.6 GB Available)

Software: iOS 8.4.1, 112 Applications Installed

An unfamiliar app icon appeared in the first unoccupied grid position on one of the home pages. The icon consisted of a red chicklet with a much lighter colored partial circle graphic and no app name text. I tapped and held one of the other app icons until they all started to dance. The unfamiliar icon did not display the Delete (Circle-X) feature. I pressed the Home button to terminate the Edit mode. After powering down and rebooting the iPad, the unfamiliar icon is not displayed on any of the Home pages. I never knowingly launched the app, if indeed there was an app associated with the icon.

Has anyone else observed this artifact?

Does anybody know what it is?

Is it malware?

Is it a manifestation of an iOS bug?

Are there any known residual consequences?

iPad, iOS 8.4.1, null
