The issues you describe could be due to improper installation of BC Drivers package. Can you check Bootcamp.xml on your USB Installer and look for Product Version entry?

I followed exactly the istructions above before asking support here but there should be some mistake because 5.1.5640 is not working.
I checked the xml and the version is 5.1.5640.
Any other suggestion?

sudo fdisk /dev/disk0
"sudo" will prompt your for your password and will warn you about improper use.
