When holding your iPhone towards the contactless reader to make your payment, might you also be holding a contactless payment card within range of the reader at the same time? For example, if your iPhone is within a wallet-style case that also contains credit or debit cards?

The feedback form enables you to report issues with specific merchants, transactions and payment cards, so is helpful in enabling Apple to investigate.
Your card issuer may also be able to investigate the matter from their side if you contact them.
