If I understand correctly, the Health app isn't responding or working as expected on your iPhone 5. Take a look at the article below and try running through the general app troubleshooting suggestions.

If an app you installed unexpectedly quits, stops responding, or won’t open - Apple Support

Restart the app

Close the app and open it again.


Restart your device

Turn your device off and back on.


If the issue persists, try restoring your iPhone as shown here:

The Health app was installed along with an iOS upgrade and there is no way to reinstall it. I have an iPhone 6, iOS 9.1 and have been using the Health app for over a month along with my Apple watch. It has been working fine. This morning the app is no longer working. It will not synch data with my watch. It indicates all of my personal data (age, height, weight and date of birth) are not set. I enter the information, select done. When I go back in, the information is gone. I use MyFitnessPal and up until today everything worked fine. Activity tracker and workout app on watch is recording data, but not synching with Health app. I unpaired my watch and then paired it and restored from backup. Information is still not being transferred to Health app.
