Mr Barrett,

there is no template. You can, however, create one very easily.

In the first row enter "Date and Time" in column A

In the first row enter "Systolic" in column B

In the first row enter "Diastolic" in column C

in the table formatter on the right make the table have one header row:

User uploaded file

enter the date and time in column A with the corresponding measurements in columns B and C

You only have to do this once:

now select columns A, B and C:

User uploaded file

now add a scatter chart from the charts menu:

Now add data to the table as you measure blood pressure:

I have made a file that you can download here - https://drive.google.com/open?id=0B0y79HErN0jDdy1YRGhzRERxaDg

If you open it, you can save it as a template.

You fill in the systolic and diastolic and pulse values. I have included a second set of values so that you can take your pressure a few minutes apart because it changes minute by minute. The sheet will automatically work out whether your pressure is High, Pre-high, Normal or Low in the status fields.

The only fields you have to fill in are Date, AM or PM, 1st Systolic, 1st Diastolic, 1st Pulse, 2nd Systolic, 2nd Diastolic and 2nd Pulse. The chart will automatically update for you to show your readings over time.

If you save as a template, you can start with a clean table every time you fill one up, which will keep all of your history.

Hope this helps.
