Mail paper clip icon not always showing

I'm using Mail in El Capitan. In my sent box (and I think in my in box) the paper click icon denoting an attachment sometimes shows up and sometimes doesn't. This is a real pain. How do I get Mail to always show if there is an attachment in the list of emails (the column to the right of the mailboxes list and to the left of the email window)?
