Instagram saving my pics to PHOTOS even when feature turned off

I'm running iOS 9.1 but the issue I'm experiencing dates back to previous iOS as well.

I have the "SAVE ORIGINAL PHOTOS" feature in Instagram turned off. However, I noticed that after posting a copy of the photo I post shows up in PHOTOS. When I turn the feature "on", then there are TWO that show up in PHOTOS.

First, is this happening to anyone else, would love to know I'm not the only one.

I'm wondering if it's an old IFTTT recipe, or an integration with Dropbox? I've gone through my Instagram permissions and revoked any that I don't recognize, including IFTTT, etc.

Hoping for some direction.... thanks in advance.
