Can't boot into Bootcamp/deleted EFI file

I deleted my EFI files/partition and couldn't start in Bootcamp. I was able to make a new EFI partition, but I still can't start in Bootcamp. I see it as a startup disk in System Preferences, it shows up as a device in Finder, but I don't see it when I use the OPTION key to start my computer.

When I run diskutil list, this is what I get:

In Finder, do you see all your Windows files in the Bootcamp volume?

You need to disable System Integrity Protection.

1. Boot into local Recovery (Command+R).

2. Run csrutil status.

3. Run csrutil disable.

4. Run csrutil status.

5. Boot normally.

6. Run csrutil status and confirm that it matches step 4 output.

7. Run the following Gdisk steps.

Please download GPT Fdisk from http://sourceforge.net/projects/gptfdisk/ and install it. It should be version 1.0.1.

Rebuild MBR to match the new GPT information thus resetting the Hybrid MBR. Use defaults for other questions (like partition codes). The only values that need modifications are the Boot flags and step 10. Accept all other defaults that Gdisk offers. Please see thesample Q&A as an example. These steps can be repeated if you make a mistake before you get to Step 12, otherwise start from Step 1 for these steps. Step 6 has numbers which are typed with a space between the numbers. Please see the sample Q&A before you execute these steps.

P (Print list of parts)
R (Recover)
O (print current Hybrid MBR)
H (chooses Hybrid)
Partitions numbers to be hybridized: 2 3 4
Y (Good for GRUB question)
N (part 2 boot flag)
N (part 3 boot flag)
Y (part 4 boot flag make NTFS bootable partition)
O (print current Hybrid MBR)
W (Write the new MBR)
Y (Yes! write the new MBR)
Reboot

Here is sample Q&A for this section. Please notice the Press Enter/Return.

Place EFI GPT (0xEE) partition first in MBR (good for GRUB)? (Y/N): Y

Test 1 - Does Bootcamp Volume show up in Finder?

Sorry for the late, late response, and thanks for your help! The Bootcamp volume shows up in Finder and I can see files in the Bootcamp volume. I can select Bootcamp in System Preferences -> Startup Disk. However, I still have a problem: when I restart the computer, it doesn't actually go into Bootcamp and instead goes back to OS X. However, if I start my computer and hold the option click, I am able to select Bootcamp and start it that way.

Why doesn't my computer start in Bootcamp even though I select it as the startup disk in System Preferences? Do you know how to fix this?

Can you look at Boot Camp: Set the default operating system and test the steps to check if it works the way you want it to?

In Administrator Tools -> Services, do you see any Apple services installed? If not, can you download BC drivers for your specific year/model and Windows version, and run the setup.exe in Bootcamp folder to repair BC drivers.

It would be good to know your Mac year/model and Windows version.

Yes, I see "Apple OS Switch Manager" when I look at Administrator Tools -> Services.

I have a 13" MacBook Pro, late 2011 version, and I am running Windows 6 64-bit on Bootcamp. Please tell me if you need any other specs, and my apologies for a late reply!

Edited to add: Also, I did run setup.exe and that didn't add any tabs to my Bootcamp menu.
