I recently upgraded to OS X El Capitan. I cannot get the "Hide Dock" and "Hide Menu bar" functionality to turn off. The checkbox to "automatically hide and show Dock" is unchecked. Also, the checkbox for "Automatically hide and show menu bar" is unchecked. What am I missing?

In your ~/Library/Preferences folder, find a file called "com.apple.systempreferences.plist" (minus the quotes, obviously). Drag it to the trash can. Restart your computer, then open System Preferences and reset all your preferences again. This should work. 🙂

I deleted the file, emptied the trash and restarted, but the system is still performing the same way. Is there a similar setting for each application, like Safari for instance?
Well yes, there's a .plist file for Safari, but Safari has nothing to do with your problem. The dock/menu bar is dealt with in System Preferences.

Have you tried checking those checkboxes, rebooting and then uncheck them?
Also you can try booting into the Recovery volume (boot with the Command + R keys held down), select Disk Utility, repair the disk and reboot normally. This also repairs permissions.

When looking at the desktop, the menu bar and dock now stay unhidden, but they still hide when I have an application open, like Safari. Is that a setting that can also be changed?

It sounds like you're opening Safari if full screen mode. Hit the Escape key when in Safari and see it that's the case.
