The last time I tried to synch my iPhone 6S to my iTunes on a Mac mini, I received the message Attempting to copy to the disk "Macintosh HD" failed. An unknown error occurred ( -54.) After clicking ok, the status shows that my iPhone was backed up but my confidence level is low given the error message I received. Any suggestions on how to resolve this? Thanks...

From an OS9 reference, error -54 is a software lock on a file or a permissions error. (Although there is no formal list of OSX error code some of the old codes still apply to OSX .)
First, try simply restarting your computer.
Try quitting iTunes, then deleting the .xml (not .itl !) version of the library file in the iTunes folder (iTunes will generate a new one).

In El Capitan I couldn't do the "repair disk permissions" as such, but I carried out the "first aid" that is offered in Disk Utility now, and then followed msg-'s steps.

I deleted .xml file and it completely cured my iTunes issues... error message just started showing up, it would take a very, very long time syncing my photos and stuff, and now it is back to quick and normal again. Thanks!
