iCloud security questions

Having discovered that my iPhone calendar was missing an entry that was in my iMac and iPad calendar I went to settings/iCloud. I was asked two security questions, neither of which I have ever been asked before nor have I ever set them up. How has this occurred and how can I put this right?

iMac (21.5-inch Mid 2010), OS X El Capitan (10.11)

Hello orchardsparks,
I understand that you are seeing questions that you don't know the answers to. That is something I can help you out with.
Typically those questions are part or the Apple ID creation process referenced here. Also those can be changed by either you or someone else that would have access to your account.
You can fix this as long as you have a rescue email setup. If you can log in at My Apple ID and navigate to the Password and Secruity tab, you can send an email to have those items reset. If you do't have on set up, then you will need to contact Apple Support for further assistance. Take a look at the link below for more detail.
