Help diagnosing a persistant problem

Hi all


I'm having loads of issues with my MacPro 6,1 (3GHz 8 core, 64 GB ram, AMD FirePro D700, 10.10.5)


On boot I get the blurred out desktop log in screen background with no box to put in my log in info (see picture)


After hard reseting the system the screen is repeated. The only way to bring up the log in box is by doing a PRAM reset


I am also experiencing regular crashes and freezes.


I have had the system in for repairs on 4 occasions (twice for this problem alone). I have had one graphics card replaced, the other graphics card had to be reconnected to the io board (as the Apple diagnostics showed that it wasn't plugged in!!) and the last repair was ordered by Apple after my Mac crashed in safe mode whilst on the phone - the company returned my mac telling me there was nothing wrong with the hardware. However on he first boot of my system the problem was still present.


I spoke to Apple again last night and they are just trying to get me to run through steps that I have already completed and talking about sending it off to be looked at again. I'd like to avoid this personally as my rental costs are getting into the thousands and there never seems to be any resolution of the issue, so I have posted the most recent crash log in the hope that someone might be able to help me.

What monitors and other peripherals are connected?

Have you tried with all unnecessary peripherals disconnected?

Have you tried the obvious of formatting the intern storage and doing a fresh install and only restoring data from backup

The Raid may be part of the problem


Also, the blurred background before the login is normal, at least for me

I have quite a few things connected -

Apple Cinema Display

Samsung 4K monitor (over thunderbolt)

OWC Thunderbay 4 (formatted as a Striped RAID set)

WD 1TB folder for non essential folders and media

USB hub

USB keyboard


Yes I have tried with just the keyboard connected and the problem still shows.


I have also done two full format installs in recovery mode in the past few months and performed fresh installs of all programs and apps.


Just one thing from your post, the blurred background is normal? You also get this and have to switch off and do a PRAM reset to get the log in window to appear?

The report you posted shows a kernel panic with no third-party kexts loaded. This should not occur in a properly-functioning Mac Pro with no Hardware problems. You should use this kernel panic as evidence that graphics problems are still occurring.


If you could replicate the problem at will, that would get you prompt resolution.


Because external devices need to transfer data very quickly, most have Direct Memory Access hardware capable of placing large blocks of data directly into RAM. (this is why peripherals are suspect).


But your kernel panic report shows these extensions present at the "scene of the crime":

If still under AppleCare, and since you have already endured a round of sending your Mac in and having it returned "no fault found" I think you should press for additional graphics card replacements, or a swap for a refurbished unit
