unlock iphone using IMEI and Itunes

I've bought an iphone off ebay locked to O2.

O2 customer services have said I can complete an online form to request the unlock - or I can plug the phone into itunes and unlock using the IMEI number - I cannot see how to do this.

Can anybody help?
