I need to get my daughters iPhone reactivated, but it is locked and we don't know the apple account or password.

IF it is her own Apple ID and password that she can't remember, try this.
https://iforgot.apple.com/appleid
If you forgot your Apple ID password - Apple Support

If she bought the phone from another person, she will need to contact them and have them erase the device for her. Pass this information along to the original owner.

If the original owner can not be contacted, your daughter owns a useless brick because there is no way to unlock it without the original owner's help.
