iTunes creates multiple playlists

I use the most updated software Apple puts out there. iTunes is set to update my phone and iPods automatically when connected, and over wifi. The playlists aren't exactly duplicates, each additional list made has less and less of the original songs in them. I do not have Apple Music or Match set up. Just normal downloads and ripped music.

The frustrating part is trying to figure out which playlist is the correct one. I've had to start from scratch on some.

Whats the deal?
