I have an iPhone 5s (Carrier: Sprint). As I am travelling out of US, I has requested Sprint to unlock my iPhone. They have provided me with a 5 digit code. But I am clueless about how and where to key in the Unlock Code.

I assume they have provided you with the instruction. Contact your carrier one more time and ask them where you should put those numbers.

This is the response from Sprint:
"I can give you the Master Subsidy Lock code that you can give to the next carrier. That carrier can attempt to use this code to unlock the device."
They gave me the MSL code.
I had taken a backup, erased the iPhone, inserted a GSM SIM card and tried to restore the backup. But after inserting the new SIM, it says that "The SIM card inserted in this iPhone does not appear to be supported. Only compatible SIM cards from a supported carrier may be used to activate the iPhone. Please insert the SIM card that came with your iPhone or visit a supported carrier store."
Do you have any idea regarding MSL code ?
You may visit authorised centre or shop of the carrier you want to use now and ask them to use this MSL code. This is the only option that comes to my mind.
No carrier will give you an unlock code for your phone.
Also, you do realise that Sprint is a CDMA carrier and not GSM right?
Go back to Sprint again and ask them.
That is not how iPhone unlocking works.
