After updating to iOS 9.1, while the phone is in silent mode, my phone automatically switches to Normal mode then suddenly to silent mode and keeps on.. We can feel this mode change as it vibrates and is so irritating.

Based on your description of the issue (intermittent switching between Normal and Silent mode), it sounds like you may be having an issue with the Side Switch. Although this could indicate a hardware issue, the more common cause is an iPhone case that is not correctly installed or one that does not fit well. Remove the iPhone from any case that it's installed in and then test it for a while to determine if it's just the case.
