Problem adding developers to enterprise program team

I am the Team Agent on an enterprise developer program and I have invited a number of members to my team. The invites show as pending, even after the invitees accept the invite. They eventually expire without anyone ever being added to the team. I have tried using existing Apple accounts and registering new accounts but the invites never show as accepted.

Is there some step in the process that I am missing?
