That is a lot of disconnects, about every minute or so. When did this start? My first thought is interference.
For example: Bluetooth: Unreliable wireless performance may occur when USB 3.0 external devices are close - Apple Support
From there is linked: If your Apple wireless mouse, keyboard, or trackpad aren't working as expected - Apple Support which has a section, "Your mouse or keyboard intermittently stops responding."
and: Wi-Fi and Bluetooth: Potential sources of wireless interference - Apple Support
As this happens regularly hopefully you can narrow it down quickly.

I can hardly rearrange my whole office to try to address the possibility of interference. So when it comes to listening to music with bluetooth headphones...I'm out of luck.

The only other thing I can find on this that may help is
Bluetooth enabled keyboards and mice may not pair or connect consistently, or audio from Bluetooth enabled earphones or speakers may be choppy. This can happen if certain external USB 3.0 devices are too close to your Mac.
- Try moving external USB 3.0 devices further away from the computer, and away from your external Bluetooth devices. For example, if you think an external USB 3.0 hard drive may be causing an issue, try relocating the device away from your Mac or move it to another side of your Mac.
- Make sure all USB cables are connected correctly. Better shielded USB 3.0 cables may also be helpful.
Hope you get it isolated.
