I currently have 2GB x 2 Stick for a total of 4GB. I want to upgrade with an additional 8GB of Ram and want to know if I have to remove the current 4 GB or can I leave them in for a total of 12 GB

Its recommended to have matched RAM, but its not absolutely necessary. Its better to have more RAM than less but matched RAM generally.
You can use the existing one with the new one.

You can use all four slots - depending on how much RAM your Mac supports. A late 2009 should support up to a total of 16 GB.
Just make sure you match the sticks horizontally, so you have 2 x 2 GB in one row and 2 x 4 GB in the other row.
