It sounds like you're seeing a contact flash on the screen when you open the Contacts app then immediately disappears despite having already deleted the app in iCloud and in Contacts. That seems to me that theres something that needs to be refreshed there, so I would start by quitting the Contacts and the Settings apps:
Force an app to close in iOS
https://support.apple.com/en-us/HT2013301. Press the Home button two times quickly. You'll see small previews of your recently used apps.
2. Swipe left to find the app you want to close.
3. Swipe up on the app's preview to close it.
When you have done that restart the device and test the issue again:iOS: Turning off and on (restarting) and resetting

That should refresh or flush whatever cache seems to be showing that contact. Thank you for using Apple Support Communities, let us know how it goes!

Dec 7, 2015 3:09 AM in response to shian22by sparkplug888,I too have this issue. Any luck any one. I have sent the feedback to apple but chances are bleak they will respond. This issue is really irritating.
I have tried saving the same number with a new name but it would keep flashing with old name. However it contacts it would show the new name. This is crazy.

contact has been deleted, new contacts added, restarted phone and all that.
This has been happening for the past few months now
- iPhone 6S 64GB, iOS 9.2.1

Feb 4, 2016 1:37 PM in response to sterling rby Gregg_3G,I have a 6S and have had this same problem. If I am opening the phone number pad initially just the number pad comes up. When I re-open the number pad a contact that was deleted still shows up. I have tried closing the phone app. I have tried re-adding the contact and deleting. I have tried adding a new contact with the same name and deleting. I have tried adding the contact and then changing the name and the original still pops up. I have tried adding other contacts from the keypad and calling them. I have tried dialing numbers then adding them as a contact from recents. And most of all...I HAVE ALREADY TRIED TURNING MY PHONE ON AND OFF BOTH NORMALLY AND HARD RESET! I appreciate the ole' preschool try but it doesn't work. More than anything its annoying but there should still be a fix.

Just like everyone else I have tried everything from restarting the phone, backing up and factory resetting, ensuring the contact isn't in my iCloud, etc. The number only shows for a split second and then it's gone. It has nothing to do with background refresh. It is definitely a bug. It is so annoying because I've never had problems with iPhone so this is not what I'm used too. Apple needs to fix this ASAP.

May 5, 2016 1:13 PM in response to shian22by iFranAir,Hey folks, I've just work it out. It's very simply. First of all, you have to reset statistics from mobile data. Then, close the phone app and the "ghost" contact dissapears forever. It worked for me.

- Veryfy that you don't have the contact in you phone adress or other apps
- Go to Settings --> Mobile Data
- Down to the option called Reset statistics and execute it
- Close settings and close phone app
- Open phone app again
These are the stepts that I did. They worked for me.
I did other things before them. You know, creating and deleting the contact again, rebooting the device, deleting safari cache, etc.
