Notes plus button

My girlfriendd has iOS 9, and was trying to turn a list in Notes into a checklist...but the "plus" button that exposes the various formatting options isn't there! The same is true for newly-created notes. I have tried it with Predictive Text turned on (she normally has it turned off), and I have looked to no avail in the Accessibility, Keyboard and Notes settings. Does anyone know if there is some other setting that turns the options on?

ONe potential issue: she does not sync Notes via iCloud, so these are all local Notes (but I note that I still have the option on my local Notes on my phone, same model, same iOS version).
