How do I delete an app from launchpad that is faded?

The above is a picture of what I cannot seem to get rid of despite all efforts. I have tried dragging into Trash and the update does not exist in the App Store. Help?

In the App store have you checked the Purchased page?

did you try clicking and holding on the app in the launch pad until it starts shaking. click the X to remove it.

It is faded here also and I am unable to click on anything.
