The highlighting works fine and then suddenly one day when I open my preview document, the highlights and underlines that i did on certain sentences misaligned and mess my whole documents. Please can somebody help me because this is soooo annoying. And the worst part is, the misaligned highlights and underlines affect the wholes pages!

I have encountered the same problem on and off, most recently today.

I wondered why problem did not always occur. I suspected it had to do with how the pdf was created. I tried this and it seems to have fixed it:

    1) Open pdf. Use File>Revert to... to go back to version prior to highlights. You could also delete misaligned highlights manually.

    2) Print>Save pdf

    3) It seems that highlighting in the new pdf works properly.

This is not a solution, but a workaround. Unfortunately, it involves redoing all your highlights.

Question: where on your computer do you store your PDF documents? Are they stored in the cloud?

(I ask because I once had an issue like this that was partially improved by no longer storing the items in the cloud.)

    Yes, in the cloud.

I see. Given that I've had issues related to Preview + cloud services that have seemingly gone away when I removed the cloud service from the equation, I wonder if you can replicate the problem once the cloud service is out of the equation.

(I have no understanding of the exact mechanisms by which a cloud service would produce this problem; this is more of a hunch based on very limited personal evidence.)
