Let's assume that you have a MacBook Pro, as the current MacBook doesn't have a fan.

The fan comes on when the MacBook Pro gets internally hot, as it should. It gets hot when it uses high levels of processing. You can use your Activity Monitor app, located in your Applications/Utilities folder, to identify the applications that may be using excessive CPU processing. It would most likely be some kind of 3rd party software.

Your Mac uses measured temperatures from multiple sensors inside to control the fan speed. If the sensors are reporting high temperatures, the fans will speed up in an attempt to cool your Mac and avoid overheating.

Occasionally, a failed sensor reports high temperatures in error, and if you suspect this Hardware fault it should be looked into immediately.
