Q: Apple Music charges

I disabled Apple Music in the iPhone settings but I received an email letting me know of the charges. I took the trial but the service is not of my interest and I would like a reimbursement. I tried the "Report a Problem" page but it did not give me the option to report a problem for this service. It was easy to opt in, but it is not as user-friendly to opt out. Please help me.

Mac mini (Mid 2010), OS X El Capitan (10.11.1)
