Dec 12, 2015 8:32 PM in response to X.xoxo.Xby AJ397,It sounds like you have done every thing this support article recommends. At this point I would recommend making a Genius Bar appointment or visit your local Apple authorized service provider. -AJ

Dec 12, 2015 8:47 PM in response to AJ397by FishingAddict,Per AJ397's suggestion, it's time to take the phone to the Apple Store for a hardware test.
I personally had this exact thing happen to my iPhone 4S a few years ago. The Genius Bar hardware test revealed that my wireless radio had failed and the iPhone was replaced on the spot.
Just make sure to do an iTunes backup prior to your appointment in case your phone needs to be replaced. This will allow you to restore your backup if your phone is replaced under warranty.
