trying to wipe my data and screen keeps freezing up

I got the IPhone 6s and am giving away my iPhone 5.

When trying to wipe my data and the screen freeze when I enter my Apple ID Password and press ERASE. I've restarted my phone several times and the same thing occurs. What to do? Trying to mail it out today...of course :-)
