how do I get calls on my iPhone to show on my MACbook Pro?

I used to receive phone calls (and could answer them) on my Macbook Pro. Can you refresh my memory as to how to turn this feature on? I must have turned it off inadvertently.

MacBook Pro, OS X Mountain Lion (10.8.2)

I've checked the following:

I'm running El Capitan on the MAC and IOS 9.2

I'm logged into my i-cloud account on the MAC, and I-phone, FaceTime is logged into my I-cloud account

Both devices are logged into my home wireless network

Phone is set to allow calls on other devices (and even shows my MacBook Pro as a choice with the button on)

I used to see the notification on my MAC for an incoming call as shown in help.apple.com under "Use Your Mac to make and receive phone calls"

I did not do anything or turn anything off that I know of, but it just stopped working - several weeks ago

Any other suggestions?
