Q: What to do if my OS X El Capitan Upgrade fails?

Via Disk Utility:

 

OS X Base System verifies fine

 

Macintosh HD says: The disk needs to be repaired.  Click Repair Disk.

 

But Repair Disk is not clickable.

MacBook Pro, OS X Yosemite (10.10.5)

    OS X Base System is the recovery partition that you will only see in Disk Utility while booted to recovery.  OS X Base System can be ignored, it is irrelevant at this point.

     

     

    Repair Disk will be greyed out in Disk Utility if FileVault is enabled.  The Macintosh HD should also have a grey color in the side bar of Disk Utility.  Check if FileVault is enabled and unlock the disk if it is before attempting to repair.  If FileVault is not turned on, verify the disk then immediately attempt to repair.
